Co-payments are due at the time of the office visit. We accept Visa, MasterCard, Discover and American Express, along with personal checks and cash. Questions regarding fees and billing should be directed to our billing department at 703-327-1800.
Pediatric Associates of Alexandria will submit claims to your primary insurance for reimbursement. We will only submit claims for your secondary insurance if it is a state funded insurance that we currently have a contracted agreement with. For example: Medicaid, Medicare or Tri-Care Prime/Standard.
Referrals Policy
Pediatric Associates of Alexandria asks that you submit request for referrals at least five business days prior to your scheduled appointment with your child’s specialist. Please provide: the child’s full name, date of birth, parent or guardian’s name, contact phone number, name of insurance, date of appointment, referred physicians complete name and office phone number and diagnosis or reason for office visit. Once the referral is complete, the parent or guardian will be notified by phone that the referral is ready for pick-up at our office. Referrals will not be faxed.
It is the responsibility of the parent or guardian to submit a referral request at least five business days prior to the child’s appointment with the specialist. Only under extreme emergency circumstances will same day referrals be approved and completed. Referrals cannot be back dated, and in most cases, your insurance requires 24 to 48 hours notice to approve (giving us authorization).
Request Medical Records Policy
Once a Medical Records Request Form has been completed in its entirety and returned to Pediatrics Associates of Alexandria, the parent or guardian will receive a phone call, informing them of the cost to obtain a copy of the records. Payment is typically made by credit card over the phone with no additional fee. Please allow approximately two weeks from receipt of payment for this process to be complete and records in the requester’s possession.
Forms
We will complete most child care, public\private school and college medical forms within three business days. Please complete all areas on the form that you’re able to and drop off or mail the form to our office. There is a $10.00 fee for each form. Please include payment with your form. |
