Insurance

We participate with the following insurance companies:

Insurance forms are the responsibility of the patient unless we participate with your plan.

Pediatric Associates of Alexandria will submit claims to your primary insurance for reimbursement. We will only submit claims for your secondary insurance if it is a state funded insurance that we currently have a contracted agreement with. For example: Medicaid, Medicare or Tri-Care Prime/Standard.

Receipts with full information for the insurance companies and for tax records are given with each visit. Please keep these receipts since they are also your itemization for supplemental and major medical policies.

If there are any questions regarding insurance, we will be glad to give assistance during regular office hours.

Fees & Billing

Information regarding our fee schedule is available in the office or by telephone. Because of the work involved and the high cost of monthly billing, payment at the time of service is requested.

Co-payments are due at the time of the office visit. We accept Visa, MasterCard, Discover and American Express, along with personal checks. We no longer accept cash payments.
